By checking the box below, signing and submitting this form, I fully understand that I am authorizing Forbici Salon and Spa to charge the credit card on file if I should fail to cancel my appointment within 24 hours of the date for which it was scheduled. This cancelation fee will be 50% of the cost of services reserved. By signing this form you give us permission to debit your account for failure to appear without timely notification for any scheduled appointment. Repeat no shows will be charged 100% for all services booked. I agree that the electronic signature appearing on this agreement is the same as a handwritten signature for the purposes of validity, enforceability, and admissibility.
